At Chosen Care, we have one clear goal: To deliver the best person-centred holistic care for adults with a wide range of learning difficulties.

Through Person Centred approaches and planning, all services and support will be tailored to meet the individual’s needs and preferences including leisure activities, educational, financial and social, providing access to both community and mainstream resources.

A unique person centred plan which incorporates individual abilities, wishes, ambitions, preferences and aspirations will be developed with the individual users of our service, we will hold regular meetings with the people using our service in order to monitor and evaluate our services to enable each individual to achieve the best possible outcome.

We believe it is important to empower all service users to take an active role in the process and running of their home.

We are committed to working as part of a team with the family, friends and advocates of the service users. To achieve this we will regularly consult them.

Service users make use of a wide range of community resources which include local colleges, sports and leisure facilities, restaurants, pubs and clubs, and vocational opportunities.

They actively encourage a highly motivated and skilled staff team, and over see the training and development of the staff to ensure the quality of support and care.

Individuals who use our services are actively encouraged and supported to take an active role within the recruitment and selection of the staff team.
